The CMS Acute Hospital Care at Home (AHC@H) initiative delivers full hospital-level care to patients in the comfort of their homes. Using clinical teams and technology, this initiative aims to provide the same standard of inpatient care as that provided to patients within the four walls of a hospital, but with fewer complications, lower costs, and higher patient satisfaction. This initiative has served over 41,000 Medicare and Medicaid beneficiaries to date, with outcomes that often outperform traditional inpatient care. Session 1: A discussion on the some of the pitfalls in the transplant system and how they can be eliminated to better serve patients with kidney disease. Focus will be on transparency, patient autonomy and fairness.
Session 2: For more than 70 years, kidney transplantation has been demonstrated to provide life sustaining kidney function and a survival benefit over dialysis, yet annually in the United States, we perform fewer than 30,000 kidney transplants. The gap between supply and demand is vast, and as a result, the organ shortage has become an unmitigated crisis. Innovative solutions are urgently needed and one such solution has entered the realm of reality: pig-to-human xenotransplantation. The FDA has issued an IND for the first clinical trial of pig-to-human xenotransplantation for patients with kidney failure, and in so doing, paved the way for Making America Healthy Again.
