The CMS Acute Hospital Care at Home (AHC@H) initiative delivers full hospital-level care to patients in the comfort of their homes. Using clinical teams and technology, this initiative aims to provide the same standard of inpatient care as that provided to patients within the four walls of a hospital, but with fewer complications, lower costs, and higher patient satisfaction. This initiative has served over 41,000 Medicare and Medicaid beneficiaries to date, with outcomes that often outperform traditional inpatient care.  This session brings together clinicians, patients, administrators and CMS subject matter experts for a comprehensive look at the experience of providing and receiving hospital-level care in the home setting, results and lessons learned, and opportunities for the future.

Amy Carter holds a Master’s degree in Adult and Community Education, is a Registered Dietitian, and a Certified Diabetes Care and Education Specialist. As a healthcare leader, she has extensive experience managing teams of skilled nutrition and wellness professionals across prevention, acute care, outpatient care, and chronic disease management. Over ten years ago, Amy implemented Diabetes Self-Management and Education Support (DSMES) at Federally Qualified Health Center (FQHC) sites, laying the groundwork for lifestyle interventions. Her programs have since expanded to include hypertension, pediatric lifestyle, produce incentives, and the National Diabetes Prevention Program, offering over 65 group series annually across eight FQHC sites. Additionally, she is a co-author of the DSMES national standards and served a term on the Academy of Nutrition and Dietetics’ Nutrition Services Payment Committee.

Session 1: A discussion on the some of the pitfalls in the transplant system and how they can be eliminated to better serve patients with kidney disease. Focus will be on transparency, patient autonomy and fairness.  


Session 2: For more than 70 years, kidney transplantation has been demonstrated to provide life sustaining kidney function and a survival benefit over dialysis, yet annually in the United States, we perform fewer than 30,000 kidney transplants. The gap between supply and demand is vast, and as a result, the organ shortage has become an unmitigated crisis. Innovative solutions are urgently needed and one such solution has entered the realm of reality: pig-to-human xenotransplantation. The FDA has issued an IND for the first clinical trial of pig-to-human xenotransplantation for patients with kidney failure, and in so doing, paved the way for Making America Healthy Again.

Driving Quality Improvement Through Medical-Dental Integration

Oral health is a vital component of overall health and well-being. This session will explore how quality improvement can drive medical-dental integration to enhance both oral and general health outcomes.

The first presentation will discuss quality measures developed by the Dental Quality Alliance that can be used to support medical-dental integration efforts. The measures include oral evaluation during pregnancy, oral evaluation following a medical preventive service visit during early childhood, oral evaluation among adults with diabetes, and follow-up visits with a dental provider after dental-related emergency department visits among children and adults. The discussion will be framed in the context of current and proposed policies related to medical-dental integration and draw from examples of successful improvement strategies.

Related to integrating medical and dental care, the second presentation will examine the utilization of dental services among pregnant Medicaid beneficiaries using administrative claims data. Medicaid covers dental services for all children, and states have the flexibility to determine what dental benefits are provided to adult Medicaid enrollees, including pregnant persons. While most states offer at least emergency dental services for adults, less than half currently provide comprehensive dental care. There are no minimum requirements for adult dental coverage. This presentation will demonstrate the importance of integrating dental care alongside pregnancy and post-partum coverage.

Finally, the third presentation will examine dental service utilization among Medicaid/CHIP beneficiaries with intellectual and developmental disabilities (IDD). These populations often need different assistance to access dental care. This presentation will highlight the differences in the use of dental services and visiting the emergency department for non-traumatic dental conditions amongst the IDD population.
